Chad D. Blackak

Agawam- Chad David Blackak 49, passed away unexpectedly at home on March 1, 2021.  Chad was born in Charleston, South Carolina the Beloved son of Matthew D. Blackak and Deborah A. (Dubour) Blackak of Feeding Hills. A lifelong resident of Agawam, he graduated from the Agawam School System.  While at Agawam High School Chad excelled in both soccer and wrestling. He received awards in both Western Massachusetts and State championships in Wrestling. He later coached Wrestling at the High School.Chad graduated from AIC with a BSN. Early in his career he found joy in Childhood Adolescent Behavioral Health. Chad was employed as a Nurse Manager for 19 years at the institute of living in Hartford. While there, he proudly received the Florence Nightingale Nurse of the Year Award. Chad was currently employed at the Brattleboro retreat as a nurse manager in adolescence. In addition to his parents, Chad leaves his daughter Lauren and son Kade Blackak. He also leaves Sandra (O’Keefe) Blackak with whom he shares his children.  A sister Kerry Smith and her husband Gerald F. Smith Jr. and their children Aidan and Laine all of Feeding Hills, his grandfather Matthew “Poppa” Blackak and his partner Gail, his loving partner Hannah Provost and her children, Lauren and her husband Gabe DeGray, and Taylor Provost.  He will be sadly missed by many special uncles, aunts, cousins, nieces, nephews and many many lifelong friends. Chad was predeceased  by his maternal grandmother Alice “Memo” Dubour, grandfather, Everett Dubour; and his paternal grandmother Lucille “Gam” Blackak Most recently Chad and Hannah have enjoyed cooking, sports, traveling and most of all time with their families. Family and friends may gather  from 12:00 NOON until 2:00 PM  on Saturday March 13,2021 at the Colonial Forastiere Family Funeral Home, 985 Main St., Agawam .  MASKS/ SOCIAL DISTANCING AND PLEASE FOLLOW THE DIRECTIONS OF THE FUNERAL HOME STAFF IN REGARDS TO VISITOR CAPACITY IN THE FUNERAL HOME.The funeral ceremony to follow is private.   LIVE STREAMING  of the funeral ceremony available beginning at 1:45PM .
